For New Patients


Prior to your first appointment, please download, complete and bring with you the following PDF forms. These forms can be filled out on your computer by opening them in Adobe Reader (please click the Adobe icon to download the program for free).


We accept appointments to treat adult endocrine disorders. To schedule an appointment with our office, please call 214-369-5992. We will mail you a ‘New Patient Packet’ which needs to be completed and brought to your first appointment along with the following: picture identification, insurance card, and referral if required. You may also download the 'New Patient Packet' by clicking on the the 'Patient Registration Form' in the right hand column. Please contact your referring physician and ask that they forward any medical records relevant to your condition such as; laboratory reports, x-rays, ultrasounds, visit notes. This may require a signed release of information. Those records can be faxed to our secure fax line at 214-369-2414.

Your appointment will be confirmed via our automated system 3 days before your appointment. If you are unable to keep your scheduled appointment, we ask that you call and notify the office so that we can accommodate another patient. Existing patients who miss an appointment or cancel with less that 24 hours notice will be charged a cancellation fee of $25. New patients who miss an appointment or cancel with less that 48 hours notice will be charged a cancellation fee of $50.

North Texas Endocrine Center is exclusively an office based practice so if your medical condition requires a hospitalization, a hospital based physician will be assigned to you. Following your hospitalization, and during your transition to outpatient care, our office will be informed of any changes in your condition, so that we may continue to manage you effectively when you return to the office.

Financial Policies

We participate in most major private insurance plans and Medicare. Co-payments are expected at the time of service. Please understand that you will also be responsible for any deductibles, coinsurance, and/or other costs not covered by your insurance plan. If a referral is required for your visit, it is your responsibility to obtain the referral before your appointment. We offer discounts for self-pay patients who pay in full at the time of service.

Medication Refills

We prefer that medication refills be handled at the time of your visit. If you need medication refills outside of your visit, we ask that you contact your pharmacy directly and have them to fax the appropriate refill request to our office at 214-369-2414. If you need to speak to a staff member about a medication refill, please call the office at 214-369-5992 and leave a message on the medical assistant voicemail. Please allow 48 hours for all medication refill requests to be completed.
